Use this page to:
1. Become a new registered user and create your new Mediabox application, or
2. If you have already been funded, log back into your application to complete a Progress Report or your Final Report, or
3. Log on using your existing user name and password (from a previous application) to create a new Mediabox application. |

The Mediabox online forms website provides you with a simple way to apply for Mediabox grants. The website helps you fill out the forms, and can check you have filled them correctly. |
|
|
|
You can log on and fill in your form at any time-all information entered is saved between visits. |
|
|
|
You MUST print off the guidelines and refer to them as you complete your online form. We have provided a version of the application in Word for you to use as a template-please note that only applications made on the online form can be accepted. |

If you experience difficulties with using this online form - please call the Mediabox team on 0121 224 7511 as soon as the problem occurs. In the few days leading up to each deadline we are particularly busy so please allow yourself adequate time to complete this part of the process. Online applications will close at exactly 2.00pm on the published deadline date. |
